It's a modified version of an existing fall prevention program. You might be able to join if you are 65 or older, live in the community, and recently visited the emergency department (ED) after a fall that wasn't caused by a serious illness or external force. The study wants to see if telehealth physical therapy and pharmacy consultations can be done in the ED and if eGAPcare can reduce how often people fall again within 6 months. This study plans to enroll 40 participants, but its current status is unclear.","Participants will be followed for 6 months after their initial emergency department visit to track falls and healthcare usage.",117,"You would receive telehealth physical therapy and pharmacy consultation while in the emergency department.
