Continuous Positive Airway Pressure or Deep Inspiration Breath Hold for Lung Cancer Radiation

This study is looking at two ways to reduce tumor movement during radiation therapy for lung cancer: using a Continuous Positive Airway Pressure (CPAP) device or a Deep Inspiration Breath Hold (DIBH) technique. The CPAP device blows air into your lungs through a mask to help expand them. DIBH is when you actively hold your breath. Researchers want to see if CPAP works better than DIBH at keeping tumors still during stereotactic body radiation therapy (SBRT). You could be eligible if you are an adult receiving SBRT for lung cancer at MD Anderson and don't have reasons not to use a CPAP device. The main goal is to measure how much tumor movement is reduced by each method, up to 10 days after starting.

Study design
This is an interventional study with 30 participants. It compares two methods of managing tumor motion during radiation therapy.
What's involved
You will undergo CT simulation scans with free-breathing, DIBH, and CPAP. If CPAP is chosen, you will wear the device for over an hour before and during your SBRT treatment.

Eligibility criteria

Inclusion

The patient will receive stereotactic body radiotherapy in the Thoracic Radiation Oncolcogy Service at MD Anderson.

Exclusion

The patient has a contra-indication for using a CPAP device.
The patient has not signed a study-specific informed consent for this study.
The patient is uncooperative.
The patient has reduced consciousness.
The patient has sustained trauma or burns to the face.
The patient has undergone any facial, esophageal, gastric or sinus surgery within the last 3 months.
The patient has idiopathic pulmonary fibrosis (IPF) as documented in their medical history.
Adults who are unable to consent, individuals who are not yet adults, pregnant women and prisoners will be excluded from this study.
  • Estimation of decrease in tumor motionUp to 10 days

    Paired t-test will be used to compare continuous positive airway pressure (CPAP) to deep inspiration breath hold (DIBH) in the decrease of tumor motion from free breathing. Linear regression may be used to evaluate the difference between CPAP and DIBH in the decrease of tumor motion from free breathing with the adjustment of important demographic and clinical variables in the model.
