Central Sleep Apnea: Physiologic Mechanisms to Inform Treatment

This study is looking for Veterans with central sleep apnea (CSA) to understand how different treatments work. CSA is a breathing problem during sleep, often seen in people with heart failure or those using opioid pain medicines. Current treatments like CPAP (continuous positive airway pressure) don't always work well enough. This study will test combination therapies, meaning you would receive PAP therapy along with one of three medications: Acetazolamide, Zolpidem, or Buspirone. Researchers want to see how these combinations affect your central apnea index (how often you stop breathing) and your CO2 reserve (a measure of your breathing control) over 120 days. The goal is to find better ways to treat CSA by targeting different parts of how your body controls breathing. All research activities are currently on hold due to COVID-19.

Eligibility criteria

Inclusion

Men and women Veterans with central sleep apnea, defined as Apnea Hypopnea Index (AHI)\>15/hour with CAHI\>5/hour, will be included in the experiments

Exclusion

less than 18 years old
pregnant or breastfeeding female
have severe respiratory disease that require to be on oxygen
recent health event that may affect the ability to participate in the study,
Body Mass Index (BMI) is \>40 kg/m2
significant insomnia
mental instability
recent health event that may affect sleep
if at any time the principal investigator (PI) identifies that a certain drug is not suitable, or are unable to use the device that is used to treat sleep apnea, will be not be allowed to participate in the study
