Real-Time Monitoring of Heart Failure at Yale New Haven Health System

This study is observing patients with heart failure (when your heart can't pump enough blood to meet your body's needs) who are already being seen within the Yale New Haven Health System. It's looking at how many people are hospitalized for heart failure and the types and doses of medicines they receive over 10 years. The goal is to better understand heart failure and how well current treatments are working in a large group of patients. You can join if you are 18 or older, have a heart failure diagnosis, and are a patient within the Yale New Haven Health System. The study is currently unclear on its recruitment status.
