This study is looking at a personalized plan for monitoring and treating patients with Familial Adenomatous Polyposis (FAP) who have already had surgery to remove their colon or part of it (proctocolectomy or colectomy). FAP is a condition where many polyps grow in the colon and rectum, which can lead to cancer. The study wants to see how well this personalized plan works to prevent advanced polyps from developing over up to 5 years. You may be able to join if you are 18 or older, have a diagnosis of FAP (either genetic or clinical), and have had a prophylactic (preventative) colectomy or proctocolectomy. The study aims to enroll 1000 participants.

- Advanced neoplasiaUp to 5 years
Incidence of advanced neoplasia (advanced adenoma and cancer). An advanced adenoma is defined as size ≥ 10mm and/or high-grade dysplasia. This surveillance and intervention protocol will be considered successful when the incidence of advanced neoplasia is less than 5% after a study period of 5 years.
