LINKED-HEARTS Program for High Blood Pressure and Other Chronic Conditions
This study is testing a program called LINKED-HEARTS for people with high blood pressure (hypertension) and either type 2 diabetes or chronic kidney disease. This program helps you monitor your blood pressure at home using a special app (Sphygmo). It also includes support from community health workers who offer education and lifestyle counseling, and pharmacists who work with your doctors to improve your medications. The study aims to see if this program can help control blood pressure after 12 months. You may be able to join if you are 18 or older, have uncontrolled high blood pressure, and either type 2 diabetes or chronic kidney disease. The study is currently recruiting 425 participants.
