Pediatric Medication Therapy Management Trial for Children with Complex Conditions

This study is testing an intervention called Pediatric Medication Therapy Management (pMTM) to see if it can help identify and manage medication-related problems in children. pMTM involves a full review of your child's medications, making sure they are optimized, and creating a medication action plan. You may be able to join if your child is between 2 and 18 years old, has at least one complex chronic condition, takes five or more medications, and receives primary care at Children's Hospital Colorado. The study will measure success by looking at the number of medication-related problems after 90 days. The current recruitment status for this study is unclear.

≥5 current medications (including prescription, as needed, and over-the-counter medications)
Receives primary care at Children's Hospital Colorado

Non-English speaking
Sibling or family member already enrolled in this study
