This study is testing a mobile application called GeriKit to better understand the health of older adults (age 75-100) who have ischemic heart disease (a condition where the heart doesn't get enough blood). The GeriKit app helps doctors assess different aspects of your health, like memory and physical abilities. We want to see if using this app helps reduce the number of times older adults are readmitted to the hospital or pass away within six months. You might be able to join if you are 75 or older, have recently been hospitalized for a heart attack, a procedure to open blocked arteries (PCI), or heart bypass surgery (CABG), can give your own consent, and can understand and perform study procedures in English. The study is currently recruiting participants.
