The COPE-AKI study is looking at a new way to care for people hospitalized with Acute Kidney Injury (AKI), which is a sudden decrease in kidney function. This study compares a special care program to the usual care you would receive. The special care program involves a study doctor, a nurse who helps coordinate your care and provides education, and a pharmacist who reviews your medications. We want to see if this special care helps you spend more days out of the hospital within 90 days of your hospital stay. You may be able to join if you are 18 or older and have moderate to severe AKI that has lasted for at least two days. The study is currently enrolling about 2145 participants.
