This study is looking at how a mobile web application (app) designed for diabetes education can help people with uncontrolled Type 2 Diabetes. The app will provide informational videos on topics like healthy eating, staying active, and managing medications. We want to see if using this app can improve your Hemoglobin A1c (a measure of blood sugar control) and your confidence in managing your diabetes. You might be able to join if you are 18 or older, have Type 2 Diabetes with an HA1c (Hemoglobin A1c) over 8.0, and are a current patient at the Family Healthcare Center, Reading Hospital Tower Health. We are looking for about 65 participants.

A validated (free to use) survey that examines self-perceived ability to manage diabetes. Scores are reported on a scale from 1-10. 1 reflecting Not at all confident and 10 reflecting confident. A higher score indicates that the individual has a better outcome, i.e., more confident that he/she can perform the task regularly at the present time.
