This study, called the Hospital to Home Transition (H2H) study, is looking for children aged 4-12 who are hospitalized for an asthma flare-up. It aims to improve how children transition from the hospital back home and address challenges in their asthma care. You and your child would be assigned by chance to either receive standard care or standard care plus support from an asthma educator/navigator for 12 months after discharge. The main goal is to see if this extra support can reduce how many times children need to be readmitted to the hospital for asthma at 3, 6, 9, and 12 months after leaving the hospital. We are currently recruiting families in the District of Columbia, Maryland, and Virginia.

Study design

This is an interventional study planning to enroll 340 caregiver-child pairs. Participants will be randomly assigned to either receive standard care or standard care plus an asthma navigator.
