Educational Approaches for Hemodialysis Vascular Access

This study is testing three different educational approaches to help people with advanced chronic kidney disease prepare for hemodialysis vascular access (a way to connect to a dialysis machine). You would be randomly assigned to one of three groups: "Education" (receiving a video and brochure), "Education-Plus" (receiving the video, brochure, and coaching sessions), or "Usual Care" (standard information from your clinic). The study aims to see if these educational methods increase the number of people who have surgery for hemodialysis arteriovenous access within 12 months or before starting hemodialysis. To join, you must be 18 or older, have had a nephrology visit for chronic kidney disease recently, and have an eGFR (a measure of kidney function) of 20 ml/min/1.73 m2 or less, or a high risk for kidney failure.
