PRESSURE CHECK: Improving Blood Pressure Control

This study, called PRESSURE CHECK, is looking for people aged 18 and older with elevated blood pressure (BP) to find better ways to manage it. You cannot join if you are pregnant, on dialysis for end-stage kidney disease, or receiving active chemotherapy. The study is comparing two team-based approaches to standard care: one uses a Remote Blood Pressure Management Program (RBPM) alone, and the other combines RBPM with support from a Community Health Worker (CHW). RBPM involves home BP monitoring and telehealth visits with a nurse or pharmacist to help manage your BP, including medications and lifestyle changes. CHWs can offer extra support with home BP monitoring, clinic visit reminders, and help with social issues like food or housing. The main goal is to see which approach is best at controlling blood pressure after 6 months. The study is currently unclear on its recruitment status.

Eligibility criteria

Inclusion

Elevated BP, defined as an average resting BP of \>=135/85 mmHg based on 3 consecutive blood pressure readings

Exclusion

People who are pregnant or who plan to become pregnant in the next 6 months at study entry
Those that have end stage renal disease on dialysis
People receiving active chemotherapy

What this trial measures

  • Blood Pressure Control6 months

    Participants will be assessed at 6 months for Blood Pressure (BP) control- defined as BP \<130/80 mmHg. To assess BP control, blood pressure will be measured in-person at the CBO (or another location) at Baseline, 6, 12 and 18 months with the primary endpoint of assessment as BP control at 6 months.
