Digital Health Navigation for Latino Patients With Type II Diabetes

This study is looking at whether special training can help Latino adults with Type 2 Diabetes use online patient portals and telehealth more effectively after leaving the hospital. The training, called Digital Navigation, involves one-on-one support to teach you how to use these digital tools for your care. Researchers want to see if this training helps you log into your patient portal within 30 days after you go home. You might be able to join if you are a Latino adult with Type 2 Diabetes, are currently in the hospital at Brigham and Women's, and speak English or Spanish. You cannot join if you are critically ill or have conditions like dementia that would make it hard to participate.

Study design
This interventional study plans to enroll 84 participants. It compares patients who receive Digital Navigation training to those who receive standard educational materials.

Self-identified Latino adult patients with type II diabetes
Admitted to general medicine floors at Brigham and Women's Hospital
Preferred language: English or Spanish

Exclusion

Critical illness
dementia, delirium, or other condition that limits ability to participate in education and consent process
