If your A1c (a measure of blood sugar over time) is 7.5% or higher, you might be eligible. The study will compare two different ways peer coaches connect with participants: weekly or bi-weekly. Researchers want to see if peer coaching helps with diabetes self-management and which coaching approach works best. The main goal is to see if your A1c changes after 6 months of coaching, and then again 3 and 6 months after the coaching ends. This study plans to enroll 76 participants.
