Comparing Laser Technologies for Kidney and Ureteral Stones

This study is comparing two ways to use a high-powered Holmium:YAG laser for treating kidney and ureteral stones (stones in the tube connecting the kidney to the bladder). Both methods use "dusting," where the laser breaks stones into tiny pieces that pass in your urine. One method uses "Dusting without Moses 2.0 Modulation" and the other uses "Dusting with Moses 2.0 Modulation." Researchers want to see if one method is better at reducing operating room time and increasing the "stone free rate" (meaning no stones are left) 6-8 weeks after the procedure. You may be able to join if you are 18-90 years old, need ureteroscopy and laser lithotripsy, and have kidney or ureteral stones between 8 and 20 mm. This study is currently recruiting 150 participants.

Study design
This is an interventional study where participants are randomly assigned to one of two treatment groups, like flipping a coin. Neither you nor your doctor will choose which treatment you receive.

On the day of surgery, you will receive one of the assigned laser treatments. You will have follow-up appointments with a renal bladder ultrasound (RBUS) and Kidney Ureter Bladder Abdominal X-ray (KUB) 8 to 12 weeks after surgery.

Eligibility criteria

Inclusion

Undergoing ureteroscopy and laser lithotripsy
Stone size ≥8 but \< 20 mm in the proximal ureter or kidney. Multiple stones ≤4 are allowed. Bilateral surgeries are allowed
Willing to sign informed consent

Exclusion

Staged surgery

  • Stone Free Rate6-8 weeks post procedure

    This is measured by KUB and Renal bladder ultrasound that is post-operative standard of care. Stone free is defined as no stone fragments seen on KUB or RBUS. If there are hyperechoic fragments seen on RBUS but not KUB and the stone analysis was not uric acid, the patient will be considered stone free. The investigators will also calculate a clinically stone free rate (\< 4mm residual fragments allowed). The Stone Free Rate assessments by the investigators will highlight the rate, or ratio of patients who are stone free and patients who are not stone free after procedure.
