Intensive Patient Referral and Education Program for Advanced Chronic Kidney Disease
This study, called iPREP-RRT, is testing an intensive education program for African American patients with advanced chronic kidney disease (CKD) and high blood pressure (Hypertension). The program includes in-person and written education about CKD and treatment options like renal replacement therapy (RRT), motivational interviewing, and home blood pressure monitoring. You'll also receive text messages to help you manage your CKD. Researchers want to see if this program improves your knowledge about CKD, your ability to manage your health, and your planning for RRT. This study is for African American adults between 18 and 70 years old who are hospitalized with advanced CKD (Stage 3B or above, or eGFR less than 45). The current recruitment status is unclear.
