This study is looking at how continuous glucose monitors (CGMs) like FreeStyle Libre 2 or Libre 3+ can help people with Type 2 Diabetes manage their blood sugar. You might be able to join if you have Type 2 Diabetes, an A1c (a measure of average blood sugar over 2-3 months) of 8% or higher, and a compatible smartphone. The study wants to see if using a CGM with guidance from a pharmacist helps lower A1c levels more than just using a regular blood sugar meter with pharmacist guidance.
