Home Heart Failure Palliative Care in Rural Appalachia

This study is testing a special program called HF-FamPALhomeCARE for people with advanced heart failure (NYHA Class III or IV) and their family caregivers in rural Appalachia. The program involves nurses providing home visits and phone calls to help manage heart failure care at home, especially for end-of-life planning. Researchers want to see if this program improves the patient's quality of life and the caregiver's well-being, and reduces depression and anxiety for patients. You might be able to join if you are 50-80 years old with advanced heart failure, and your caregiver is 45-80 years old. The study aims to enroll 208 participants.

Study design
This is a randomized controlled trial, meaning participants are randomly assigned to receive either the HF-FamPALhomeCARE program or standard care. The study aims to enroll 208 participants (104 patient-family caregiver pairs).

If you are in the HF-FamPALhomeCARE group, you would receive two home visits and three bi-weekly phone calls about heart failure home care, plus follow-up phone calls at 3, 6, 9, and 12 months. Your quality of life will be measured at the start, and at 3 and 6 months.
