Activity and Healing in Pediatric Forearm Fractures

This study is looking at how different activity levels affect the healing of forearm fractures (broken bones in the arm) in children and teenagers aged 8 to 18. We want to see if being more active while in a cast increases the chance of the bone moving out of place again (re-displacement). Participants will be randomly assigned to either receive "Restricted Activity Recommendations" or "Full Activity/Limited Activity Recommendations" (no contact sports). The main goal is to see if the bone re-displaces during the 6-8 weeks the cast is on. We are also looking at other complications like skin irritation or needing another surgery. You may be able to join if you have certain types of forearm fractures.

Study design
This is an interventional study planning to enroll 100 participants. Participants will be randomly assigned to one of two activity recommendation groups.

Eligibility criteria

Inclusion

Isolated Distal Radius Metaphyseal Fx (with or without ulna styloid)
Distal Third (\<4 cm from physis) Radius and Ulna fracture (i.e. without obvious physeal involvement)
Insolated Radial Shaft Fracture (diaphyseal)
Radial and Ulna Shaft Fracture (diaphyseal)

Exclusion

Initial presentation \>7 days from the time of injury
Pathologic fracture
Any patient with metabolic bone disease (ex. Osteoporosis, skeletal dysplasias)
Any patient with known bone fragility condition (ex. Osteogenesis imperfecta)
If operative treatment is required at initial presentation

    Re-displacement will be defined as a change in angulation of greater than 10 degrees.
