Shoulder Arthroplasty and C. Acnes Infection Prevention

This study is looking at different washes applied after the first cut in shoulder replacement surgery to see if they can lower the amount of *C. Acnes* bacteria. *C. Acnes* is a common skin bacteria that can sometimes cause infections. You could be in one of four groups: receiving Xperience Antimicrobial Wash, 3% hydrogen peroxide, 10% povidone-iodine (Betadine), or no wash after the initial incision. Researchers will take bacterial cultures to see if these washes reduce *C. Acnes*. To join, you must be at least 18 years old, need a primary shoulder replacement, and have tried at least 6 weeks of other treatments. The study aims to see if these washes lead to fewer positive *C. Acnes* cultures up to 18 days after surgery. The current recruitment status is unclear.

Eligibility criteria

Inclusion

Indicated and scheduled for primary shoulder arthroplasty.
Chronic severe glenohumeral osteoarthritis, avascular necrosis, post traumatic osteoarthritis, rotator cuff arthropathy,
Ability to read and understand English
Age ≥18 years
Patient failed ≥6 weeks of conservative treatment, which included non-steroidal anti-inflammatory drugs (NSAIDS)

Exclusion

Patient with history of prior native shoulder septic arthritis or infection
Prior surgery of affected shoulder
Proximal Humerus Fracture
Active infection
Cancer
Autoimmune and rheumatologic disorders, including rheumatoid arthritis, psoriatic arthritis, systemic lupus erythematosus
History of allergic reaction to citric acid-derived products
  • Incidence of C. Acnesup to 18 days

    C. Acnes cultures will be sampled from surgical site prior to the initial incision (skin culture), from the incision site after the initial incision (dermal culture), from the glenohumeral joint (glenohumeral joint culture), and from the room air (air culture). Each culture will be monitored for up to 18 days for growth of C. Acnes.
