Re-Engineered Discharge for Diabetes Care Transitions
This study is testing a new way to help people with type 2 diabetes who are in the hospital and have unmet social needs. Social determinants of health (SDOH) are things like housing, transportation, and access to healthy food that can affect your health. The study will screen patients for these needs and then offer support after they leave the hospital. You could be eligible if you are an adult (18 or older) with type 2 diabetes, are hospitalized at a UMass-affiliated hospital, and report at least one unmet social need. You cannot be pregnant, discharged to a nursing facility, or have certain medical conditions or cognitive impairment. The study aims to see if this support, called REDDCAT2 Post-Discharge Navigation, can reduce how often people need hospital services after discharge, compared to standard care.
