Postpartum Ultrasound Evaluation for Hemorrhage Risk

This observational study is looking at whether a postpartum ultrasound exam can help predict a type of excessive bleeding after childbirth called "postpartum hemorrhage." This type of bleeding can happen between 1.5 and 24 hours after delivery. Researchers will perform an ultrasound exam about 1.5 to 2 hours after you give birth to measure your uterus, endometrial stripe (lining of the uterus), and check for fibroids (non-cancerous growths). They will also collect information about your medical history, labor, and delivery. This study aims to see if these ultrasound measurements can identify women who might be at higher risk for this "medium-term" bleeding. They plan to include 1725 pregnant women who are planning to deliver at Endeavor Health, are at least 37 weeks pregnant, and speak English.

Inclusion

Currently pregnant and planning to deliver at Endeavor Health \>37 weeks gestational age
English speaking
Labor \<6cm dilated at time of consent without epidural
Labor \<8cm dilated at time of consent with epidural

Exclusion

Preterm delivery
Non-English speaking
Any conditions that impairs potential participants decision making

    Width (cm) of the endometrial stripe (measuring displacement of the uterine walls by intrauterine contents (blood, clots, retained placental fragments or other) to include Doppler study) at 3 levels: * Fundal * Lower uterine segment * Cervix These will be correlated with subsequent clinical outcomes, including bleeding events.

    Presence of uterine fibroids (number and size in cm of each) These will be correlated with subsequent clinical outcomes, including bleeding events.

    Caliber of the vagina (cm) (to assess distension by clot) These will be correlated with subsequent clinical outcomes, including bleeding events.
