Remote Peer Navigator Intervention for Adults with Newly Acquired Disabilities

This study, called OP-ENS - CL, is looking at whether a remote peer navigator program can help adults with new physical disabilities as they transition back to community living. You could be eligible if you're 18 or older, have a physical disability from a neurological condition (like a spinal cord injury, stroke, or Parkinson's disease) that started in the last 6 months, and have moved back into the community within the last 4 months. The program involves connecting you with a trained peer navigator who also has a physical disability. They will offer support and guidance through conversations, goal setting, and action planning over 12 months. Study design
This study plans to enroll 100 participants who will be randomly assigned to either receive the OP-ENS - CL intervention or usual care.
