T1CARE: Addressing Social Needs in Young Adults with Type 1 Diabetes

This study, called T1CARE, is testing a new way to help young adults (ages 18-40) with type 1 diabetes. It focuses on addressing social determinants of health (SDOH), which are things like housing, food, or transportation that can affect your health. The T1CARE intervention involves a Community Health Navigator (CHN) who will help you with these needs, coordinate your care, and offer social support. We are looking for 100 participants in the greater New Haven area who have type 1 diabetes and are fluent in English or Spanish. The main goal is to see if T1CARE helps resolve these health-related social needs within 6 months. The study status is currently unclear.

fluent in Spanish or English who reside in the greater New Haven area

cognitive impairment and otherwise unable to give written informed consent about participating in this study
