This study is testing a new software called the Patient Centered Multi-Agent Decision Support System (PCM-DSS) for healthcare providers treating people with type 2 diabetes (T2D) who are using multiple diabetes medications. You would be using a Continuous Glucose Monitor (CGM) along with either this new PCM-DSS software or standard care. The main goal is to see how well the PCM-DSS works to improve your health over 22 weeks. To join, you need to be at least 18 years old, have type 2 diabetes for at least 6 months, have an HbA1c (a measure of blood sugar over time) of 7.0% or higher, and be using a long-acting insulin like insulin glargine or insulin degludec for at least 3 months. This study is currently unclear if it is recruiting, but plans to enroll 18 participants.
