Duke Cardiometabolic Prevention Clinic for Heart Disease and Diabetes

This study is looking at whether a special team-based clinic at Duke can help people with type 2 diabetes and heart disease better manage their blood pressure and cholesterol. The clinic, called the Duke Cardiometabolic Prevention Clinic, brings together heart doctors, kidney doctors, diabetes specialists, and liver doctors. You might be able to join if you are an adult (18 or older) with a history of heart disease and type 2 diabetes, and your blood pressure (systolic blood pressure or SBP) and bad cholesterol (LDL-C) are not well controlled. The study aims to see if this clinic improves blood pressure and cholesterol control over 12 months, increases the use of recommended heart medications, and reduces hospital visits.

Study design
This interventional study plans to enroll 150 participants. You will be randomly assigned to either receive your usual care or be referred to the Duke Cardiometabolic Prevention Clinic.

SBP \> 150mmHg on at least 1 occasion in last 3 months, AND
LDL \> 130mg/dL in last 3 months
