CTraC-FIT for Veterans with Functional Impairment
This study is developing and testing a new program called CTraC-FIT to help older Veterans who are at high risk of being readmitted to the hospital and are experiencing functional impairment (difficulty with daily activities). CTraC-FIT aims to improve their ability to do daily tasks and maintain independence after leaving the hospital. You might be able to join if you are 65 or older, have certain scores on surveys about your vulnerability and physical performance, and can give your informed consent. The study will look at how satisfied participants are with the program and how they perceive the intervention. Enrollment is currently unclear, but the study plans to include 40 participants.
