Designing Care Management for Hospice Transitions for Persons Living With Advanced Dementia

This study is testing a new approach called the Dementia Care Management Hospice Transitions Checklist. This checklist is designed to help people living with advanced dementia and their care partners during the transition to hospice care. The study aims to see if this checklist is practical to use (feasible), acceptable to patients and caregivers, and effective in guiding end-of-life care. Researchers will also look at how often people enroll in hospice and how satisfied care partners are with the checklist. You may be able to join if you are a care partner of someone with moderate to severe dementia, or if you are a healthcare professional involved in hospice transitions for people with dementia. This study is currently unclear regarding its recruitment status, but plans to enroll 96 participants.

Study design
This is a single-arm study, meaning all participants will receive the Dementia Care Management Hospice Transitions Checklist. It is a feasibility trial, aiming to understand if the intervention can be successfully implemented.
What's involved
Care managers will have telephone conversations with care partners about care needs, using the checklist as a guide. Feasibility data will be collected within one month after receiving the intervention.
Compensation
Not stated in the trial record.
Follow-up
Feasibility data will be collected within one month after participants receive the intervention.

  • Feasibility of the Dementia Care Management Hospice Transitions ChecklistAfter enrollment and study participation, we will collect feasibility data within 1 month after intervention receipt.

The primary outcome is feasibility. Feasibility will be measured for each group including recruitment and retention rates, rate of completion of the intervention as the proportion of individuals who use and receive the intervention, and whether the different components of the intervention are achievable.
