This study is looking at whether a special one-page guide can help more people complete their Fecal Immunochemical Test (FIT) for colorectal cancer screening. FIT is an at-home test that checks for signs of cancer in your stool. The guide, called an "implementation intention guide," is designed to help you plan and overcome any difficulties in completing the test. We want to see if adding this guide makes more people send in their FIT kits. You can join if you are between 45 and 75 years old and receive primary care at specific clinics in Lebanon, NH, Lyme, NH, or White River Junction, VT, or are eligible for VA Mailed FIT. The study aims to enroll 1600 people.

Cared for by primary care at General Internal Medicine Clinic, Dartmouth-Hitchcock Medical Center (Lebanon, NH), Dartmouth Hitchcock Clinics Heater Road Primary Care (Lebanon, NH), Dartmouth Hitchcock Clinics Lyme Primary Care (Lyme, NH), or White River Junction Veterans Affairs Medical Center (White River Junction, VT) OR eligible for FIT through VA Mailed FIT Program
