Portable MRI for Brain Injury After Cardiac Arrest

This study is looking at how well a portable MRI scanner can find brain injury in people who have had a cardiac arrest (when the heart stops beating). Cardiac arrest can lead to brain injury, and doctors currently use tests like conventional MRI to predict recovery. However, conventional MRI requires moving patients, which can be risky for those on life support. This study aims to see if portable MRI, which can be used at the bedside, can accurately detect brain injury. You might be able to join if you are at least 18 years old, unresponsive after your heart restarted, and your doctor thinks a conventional MRI is needed. The study will compare the portable MRI scans to conventional MRI results to see how well they match. The goal is to make brain imaging more accessible and safer for patients after cardiac arrest.

Study design
This is an observational study planning to enroll 60 participants. It will compare portable MRI scans to conventional MRI for measuring brain injury.

You would receive 2-3 portable MRI scans: one within 6 hours of cardiac arrest, another between 6-24 hours, and a third around the time of your conventional MRI.

Eligibility criteria

Inclusion

Unresponsive immediately after ROSC from IHCA or OHCA
Age ≥ 18 years of age
Conventional MRI is clinically indicated
Treatment with temperature control

Exclusion

MRI contraindication according to the American Heart Association guidelines
Inability to tolerate supine positioning for 30 minutes
Diffuse loss of grey-white differentiation and sulcal effacement on head computed tomography within 6 hours of ROSC
  • Cortex- DGN HIBI scoreup to 6 hours post cardiac arrest, up to 24 hours post cardiac arrest and within 12 (+/- 12) hours of conventional MRI

    Hypoxic ischemic brain injury will be assessed using the Cortex- DGN HIBI score on a scale of 0-40. Higher scores indicate greater injury.
