This study is looking at ways to improve follow-up care for adults experiencing homelessness who visit the emergency department (ED). If you are 18 or older, currently unsheltered, and being discharged from the ED with certain conditions like COPD or asthma flare-ups, pneumonia, heart failure, or skin infections, you might be eligible. The study is testing two ways to help you get follow-up care: either a referral to a regular clinic or a referral to a street medicine team. The street medicine team will try to find you and provide care where you are, rather than asking you to come to a clinic. Researchers will see if these approaches reduce how often you need to visit the ED at 1 and 3 months after your initial visit. The current status of this study is unclear.
