Observational Study of Glucose Monitoring After Liver Transplant

This study is looking at how blood sugar levels change after a liver transplant, especially for people who have had high blood sugar in the hospital or have Type 2 Diabetes. It uses a device called Libre 3+ (a continuous glucose monitor or CGM) to track blood sugar. You would wear this device after leaving the hospital. The main goal is to see how much time your blood sugar stays within a healthy range over 28 days. The study is open to adults aged 18 and older who have had a liver transplant and a blood sugar reading over 180mg/dL while in the hospital. We are looking for 40 participants, and the current recruitment status is unclear.
