Urine Sodium-Driven Diuretic Adjustment Strategy in Acute Decompensated Heart Failure

This study is looking at new ways to use a common heart failure medicine called Furosemide (an intravenous solution, meaning it's given through a vein). The goal is to see if adjusting the dose of Furosemide based on your urine sodium levels can lead to better results for people hospitalized with acute decompensated heart failure (when your heart can't pump enough blood to meet your body's needs). Researchers want to know if targeting higher urine sodium levels helps remove more fluid, reduces how long you stay in the hospital, and lowers the chance of coming back to the hospital. You might be able to join if you are an adult (18 or older) admitted to Lakeland Regional Medical Center with acute decompensated heart failure and have signs of too much fluid in your body, like swelling in your legs. Study design

Eligibility criteria

Inclusion

The study will target adults 18 years of age or older admitted to Lakeland Regional Medical Center who:
Have a primary diagnosis of acute decompensated heart failure, and
Have at least one of the following signs of hypervolemia:
Bilateral lower extremity pitting edema
Jugular venous distension
Hepatojugular reflex
Crackles on pulmonary exam, or
Chest x-ray demonstrating pulmonary edema or pleural effusions

Exclusion

Admission to the intensive care unit
Presence of ST-elevated myocardial infarction
Presence of type 1 non-ST elevated myocardial infarction
Dyspnea primarily due to non-cardiac cause as judged by the emergency medicine physician or admitting physician
Use of supplemental oxygen totaling at least 3 L per minute or greater at baseline
Anuria
End-stage renal disease per KDIGO criteria
Dialysis use
