Eligibility criteria

Inclusion

No EHR-documented healthcare proxy at enrollment
Age ≥ 55 years OR age ≥ 18 years and listed in the health system's cancer, CHF, or CKD registries

Exclusion

Primary care provider not assigned at time of randomization.
