Study for Type 2 Diabetes in Immigrant Communities

This study is looking at new ways to help people with Type 2 Diabetes, especially those in low-income immigrant communities. It tests two approaches: the first is a Community Health Worker (CHW)-enabled Community-Clinic Linkage (CCL) program, which connects you with social services to help with needs like food or housing. The second is Mobile health (mHealth) diabetes self-management education and support (DSMES) videos, which are culturally-tailored videos sent to your phone. The main goal is to see if these approaches can improve your HbA1c (a measure of blood sugar control) over six months. You might be able to join if you are 18-70 years old, have Type 2 Diabetes with an HbA1c of 8% or higher, or a new diagnosis, and speak English, Mandarin, Cantonese, or Spanish.
