This study aims to improve the health of mothers and babies in Rhode Island by providing extra support during pregnancy and after birth. It focuses on helping birthing people who receive care at Women and Infants Hospital (WIH) and its clinics. Nurses will screen you at different times during your pregnancy and after birth for things that might affect your health, like social challenges, substance use, or mental health conditions. If you screen positive, a care manager and community health workers will connect you with services like doula care, help with food, housing, or transportation. The main goal is to reduce serious health problems and deaths for mothers from when you enroll until one year after birth. This study is for all birthing people receiving perinatal care at WIH and its affiliated clinics.

Study design

This is an interventional study planning to enroll 3000 participants. It is designed to compare maternal and infant health outcomes before and after the program is put in place.
