Community Health Worker Coaching for Blood Pressure After Pregnancy
This study is looking at two ways to help women manage their blood pressure after pregnancy, especially if they had high blood pressure during pregnancy. One group will use Remote Blood Pressure Monitoring (RBPM) alone, where you'll measure your blood pressure at home for 6 weeks. The other group, called the Enhanced Care Group, will also use RBPM but will additionally receive support and lifestyle coaching from a community health worker. The goal is to see if the Enhanced Care Group helps women control their blood pressure better, improves heart health, and reduces hospital visits within 6 months after delivery. You can join if you are 18 or older, are at least 28 weeks pregnant with a single baby, have high blood pressure during pregnancy, and plan to deliver at Prisma Health-Midlands Hospital. What this trial measures
- Rates of Blood Pressure Control6 months
Percentage of participants with controlled blood pressure (\<130/80 mmHg). Blood pressure will be measured three times for each participant, with 60 seconds rest between each measurement using Omron HEM-780 automated monitor. The average of three measures will be used for each visit.
