You might be able to join if you are an adult (18 or older) with ASCVD, have seen a CommUnityCare provider recently, and your "bad" cholesterol (LDL) levels are not at target (either below 55 or 70 mg/dL, depending on your risk factors). If you join, you'll receive phone calls from study staff to help coordinate appointments, review your medications and lab results, and answer your questions. Your cholesterol medications may be adjusted to better lower your cholesterol. The study will track your visits, medicines, and cholesterol levels for 6 months to see what percentage of patients reach their LDL target.
