Evaluating Talar Osteochondral Lesions in Pilon Fractures

This study is looking at patients who have pilon fractures (a type of ankle fracture involving the shin bone) to see how often they also have talar osteochondral lesions (damage to the cartilage and bone in the ankle joint). Researchers will use a small camera called an Arthrex Nanoscope during a procedure called diagnostic ankle nano-arthroscopy. This will happen when you are already having an external fixator placed to stabilize your ankle. The goal is to find out how common these lesions are in pilon fractures and if knowing about them earlier can help doctors plan the best treatment for you. The study aims to enroll 25 participants who are at least 18 years old with specific types of pilon fractures and no prior ankle surgery.

Inclusion

Age ≥ 18 years (skeletally mature) Tibial plafond fractures with impaction of the articular surface requiring external fixation for temporization AO/Orthopaedic Trauma Association (OTA) Fracture classification 43-C type fractures (excluding posterior pilon or supination-adduction type ankle fractures) Closed injury

Exclusion

Prior ankle surgery
Pre-existing ankle arthritis
Unstable polytrauma patients precluding safe temporization
Concomitant talus or calcaneus fractures identifiable on plain radiographs
Pregnant
