Real-Time Acute Kidney Injury Perioperative Prediction Clinical Trial

This study is looking at whether a special computer tool, called an EHR-Embedded AKI Risk Score, can help doctors prevent acute kidney injury (AKI) after surgery. AKI is a sudden decrease in kidney function. The tool uses information from your electronic health record (EHR) to predict your risk of AKI before surgery. Doctors will either see this risk score alone, or see it with an alert that suggests best practices for kidney protection. The study is for adults aged 18 and older who are having non-obstetric surgery at UCSF and are expected to stay overnight. We will measure changes in your creatinine levels (a marker of kidney function) from before surgery to 1-2 days after surgery to see if the tool helps. The study aims to enroll 25,518 participants, but its current status is unclear.

Eligibility criteria

Inclusion

Adults ≥18 years undergoing non-obstetric surgery at UCSF.
Inpatient cases with expected overnight stay.
Baseline eGFR ≥15 mL/min/1.73 m².
Managed by an attending anesthesiologist randomized to one of three arms (CRNAs/residents follow attending).
Data available in the UCSF EHR for risk scoring and outcomes.

Exclusion

Obstetric procedures.
Chronic dialysis patients.
Kidney transplant recipients.
Cases without baseline creatinine/eGFR or missing essential EHR elements needed for scoring/outcomes (operational exclusions).
Outpatient procedures without expected overnight stay.
