Clinician Notification to Increase Aspirin Prophylaxis for Preeclampsia Prevention

This study is looking at ways to help more pregnant individuals at high risk for preeclampsia (a serious blood pressure condition during pregnancy) receive aspirin prophylaxis (preventive treatment). Currently, low-dose aspirin can reduce the risk of preeclampsia but isn't always used. This study will test if telling doctors when their patient has a high risk for preeclampsia, and reminding them to consider aspirin, leads to more patients getting this important preventive treatment. You could be eligible if you are pregnant, 18 or older, between 12 and 28 weeks' gestation, and identified as having a high risk for preeclampsia. The study will be successful if more pregnant individuals at risk for preeclampsia receive aspirin prophylaxis between 12 and 28 weeks' gestation. The current recruitment status is unclear.

Study design

Eligibility criteria

Inclusion

Pregnant individuals receiving outpatient obstetric care at participating medical centers.
Age 18 years or older.
Currently pregnant and within 12 to 28 weeks' gestation
Identified as having elevated predicted risk for preeclampsia as defined by USPSTSF and ACOG criteria
Receiving care from a clinician, care team, or clinic participating in the randomized implementation trial.

Exclusion

Known contraindication to aspirin prophylaxis documented in the electronic health record.
Pregnancy beyond the prespecified gestational age window for aspirin prophylaxis implementation at the time of eligibility assessment.
Not receiving ongoing obstetric care at a participating site.
