This study is looking at how to best screen for and address social determinants of health (SDOH) in children and young adults with Type 1 Diabetes (T1D). SDOH are factors like your living situation, access to healthy food, or financial stability that can affect your health. The study will test a program where social workers screen patients and families for SDOH and then provide support based on the results. We want to see if this screening is documented well and if families are satisfied with the process. You may be able to join if you are 21 years old or younger with T1D, or a parent/guardian of such a child, and receive care at the UCLA Pediatric Diabetes program in Westwood. The study plans to enroll 150 participants, but its current recruitment status is unclear.
