SUSTAIN is a telephone-based service designed to help older adults manage behavioral health issues like depression, anxiety, and chronic pain. The study aims to understand what factors influence how well people respond to the program and to see its long-term effects. You may be able to join if you are 65 or older, participating in the SUSTAIN Clinical Program, and willing to give verbal consent for a 6-month outcome evaluation. Success in this study will be measured by changes in your overall health and quality of life, using a survey called the Veterans Rand - 12 Item Health Survey (VR-12) after six months.
